A missed load restraint check, an unrealistic delivery slot or an incomplete fatigue record can create exposure well beyond the driver’s cab. A practical chain of responsibility compliance checklist helps transport operators identify who can influence a heavy vehicle task, apply workable controls and retain proof those controls are being used.

For fleet businesses, Chain of Responsibility (CoR) is not a paperwork exercise completed once a year. It is the operating discipline that connects schedulers, loaders, depot teams, contractors, executives and drivers to safer, lawful heavy vehicle movements. The strongest systems make compliant work the easiest way to work.

What a Chain of Responsibility checklist should achieve

Under the Heavy Vehicle National Law (HVNL), parties in the transport supply chain must take reasonable steps to ensure their actions or inactions do not cause or encourage a breach. Depending on the activity, this may include an employer, operator, scheduler, consignor, consignee, packer, loading manager, loader, unloader or prime contractor.

A useful checklist does more than confirm that policies exist. It tests whether responsibilities are assigned, risks are identified before work begins, controls are followed in the field and records can demonstrate what happened if a regulator, customer or insurer asks.

The right level of detail depends on your operation. A concrete fleet managing agitator allocations and delivery windows faces different pressures from a long-haul carrier, waste contractor or plant hire business. The underlying test remains the same: can each person with influence over the task show the reasonable steps they took to prevent breaches involving fatigue, speed, mass, dimension, loading and vehicle standards?

Chain of responsibility compliance checklist

1. Map every party who can influence the transport task

Start with the actual workflow, not the organisation chart. Follow a job from booking through allocation, loading, travel, delivery, unloading and return to base. Record the people and businesses that influence timing, load details, route, vehicle selection, driver availability and site access.

Include subcontractors and customer-facing teams where they affect transport decisions. A customer service team that promises a narrow delivery window, for example, may create a scheduling pressure that needs to be controlled. Each role should have a documented CoR responsibility, an accountable manager and a clear escalation path.

2. Assess the risks created by normal operating decisions

Review the decisions made every day that can lead to a breach. These commonly include late changes to delivery times, unverified load weights, route changes, long site queues, pressure to recover lost time, vehicle substitutions and incomplete pre-starts.

For each risk, document the likely consequence, existing control, control owner and evidence available. Avoid generic risk statements such as “driver fatigue”. Be specific: “Scheduler assigns a late job without checking remaining work and rest hours” is a risk that can be observed, measured and managed.

This is where telematics, electronic work diaries, onboard mass systems and job management data become operational controls rather than separate technology feeds. Used together, they can show whether plans match the conditions drivers face on the road.

3. Set realistic schedules and fatigue controls

Scheduling is often where compliance and commercial pressure meet. Delivery commitments need to account for loading time, travel time, traffic, site delays, mandatory breaks, unloading and realistic contingencies. A schedule that only works if a driver skips a break or speeds is not a compliant schedule.

Check that schedulers can see driver availability and relevant fatigue information before allocating work. They also need authority to reallocate jobs, adjust customer commitments or stand down a vehicle when a safe plan is not available. If those decisions require approval, the approval process must be fast enough to work during a live shift.

Review actual versus planned journey times regularly. Repeated overruns on a route, customer site or time slot are evidence that the plan needs adjustment, not that drivers need more pressure.

4. Verify mass, dimension, loading and load restraint

Loading controls should begin before the vehicle arrives at the loading point. Confirm the nominated vehicle is suitable for the load, the load information is accurate and site personnel understand applicable mass, dimension and restraint requirements.

At the point of loading, capture the checks that matter to the task. This may include axle group weights, gross mass, load placement, restraint equipment condition, restraint method, permits and route restrictions. For bulk, concrete, waste and plant movements, the risk profile can change quickly as load characteristics, site conditions and vehicle configuration vary.

A signed form alone is not enough if the process is not followed. Periodic supervisor observations, weight data reviews and exception reporting help establish whether controls are working in practice. Where onboard mass technology is used, define who reviews alerts and what action is required before the vehicle continues.

5. Make vehicle condition and pre-starts visible

A vehicle defect can become a CoR issue when a party allows a vehicle to operate despite knowing, or reasonably being expected to know, that it is unsafe or non-compliant. Your checklist should confirm that drivers complete fit-for-service pre-starts, defects are categorised appropriately and safety-critical defects result in prompt action.

Drivers need an easy way to report faults from the vehicle or mobile device, especially outside depot hours. Maintenance teams need a workflow that records assessment, repair, release to service and any temporary operating restriction. Operations staff should be able to see whether a vehicle is unavailable before allocating it to a job.

The evidence trail matters. Time-stamped pre-starts, defect photos, maintenance records and vehicle status data provide a far stronger position than a paper process that cannot be readily retrieved.

6. Monitor behaviour, intervene early and record outcomes

Speeding, harsh driving events, route deviations, unauthorised vehicle use and missed fatigue prompts are not just dashboard metrics. They are opportunities to identify a control failure before it becomes an incident.

Set thresholds that reflect the operating environment and nominate who receives alerts, who investigates and when escalation is required. A minor event may warrant coaching. Repeated events, serious speeding or a pattern linked to a particular schedule, route or site may require a formal review of the broader system.

Driver-facing camera technology can add context where an event needs investigation, but it should sit within a clear privacy, access and retention framework. The objective is fair, evidence-based intervention, not surveillance for its own sake.

7. Train each role for the decisions it actually makes

CoR training should not be limited to drivers. Dispatchers need to understand the effect of unrealistic allocations. Loaders need to recognise when a load cannot safely leave site. Managers need to know how to respond to repeated compliance exceptions and when resources must be changed.

Maintain training records, but also test competency through real scenarios. Ask a scheduler how they would manage a delayed driver approaching a fatigue limit. Ask a loader what happens when weight information conflicts with the nominated vehicle configuration. These conversations reveal whether procedures are understood under operational pressure.

8. Control contractors and customer interfaces

Contractor arrangements do not remove responsibility. Before engagement, assess whether subcontractors have suitable vehicles, maintenance arrangements, fatigue processes, licences, insurance and reporting capability. During the relationship, review actual performance, incidents, defects and compliance exceptions rather than relying only on onboarding documents.

Customers can also influence compliance through booking requirements, site constraints and turnaround expectations. Where a customer request introduces risk, provide a defined process for challenging it, documenting the discussion and agreeing a safe alternative. Commercial teams should know that a profitable job is not one that requires a breach to complete.

9. Keep records audit-ready, not archive-heavy

Records should be easy to find, consistent and connected to the relevant task, vehicle, driver or decision. Retention periods and specific documentation requirements can vary, so align your framework with applicable legislation, contractual requirements and legal advice.

Focus on records that prove action: risk assessments, inductions, allocation decisions, work and rest evidence, mass and loading checks, pre-starts, defect closures, training, investigations and corrective actions. Turn exceptions into corrective action

A checklist has value only when exceptions change the way work is done. Establish a regular compliance review that brings together operations, safety, maintenance and leadership. Examine trends by vehicle, driver, depot, customer site, route and contractor.

Then assign corrective actions with an owner and due date. The fix may be a changed delivery window, a revised route, additional loader training, a maintenance process change or better visibility for schedulers. Close the action only when evidence shows the control is operating as intended.
